  9.     Declaration made this __________ day of ________________ 
  10. 19________. I Wyatt Cox, being of sound mind, willfully and 
  11. voluntarily make known my desire that my dying shall not 
  12. be artificially prolonged under the circumstances set forth 
  13. below, do hereby declare: 
  14.  
  15.     If at any time I should have an incurable injury, 
  16. disease, or illness certified to be a terminal condition by 
  17. two physicians who have personally examined me, one of whom 
  18. shall be my attending physician, and the physicians have 
  19. determined that my death will occur whether or not life- 
  20. sustaining procedures are utilized and where the application 
  21. of life-sustaining procedures would serve only to artificially 
  22. prolong the dying process, I direct that such procedures be 
  23. withheld or withdrawn, and that I be permitted to die 
  24. naturally with only the administration of medication or the 
  25. performance of any medical procedure deemed necessary to 
  26. provide me with comfort care.

  28.     In the absence of my ability to give directions 
  29. regarding the use of such life-sustaining procedures, it is 
  30. my intention that this declaration shall be honored by my 
  31. family and physicians as the final expression of my legal right 
  32. to refuse medical or surgical treatment and accept the 
  33. consequences from such refusal.

  36. I am emotionally and mentally competent to make this 
  37. declaration.
